New patients - How to register at Broadway Medical CentreNew patients should come to the surgery to register with us. We do not currently offer an online service for this process, though you may download the forms to save time at the surgery.

You should ideally bring proof of address (passport, photocard driving licence, utility Bill etc) and your NHS (National Health Service) number with you when you come to register. You will also be required to provide a urine sample at the time of registration. Please ask at reception for further information

You will need to be able to tell us your previous UK address and postcode, AND that of your previous GP.

You will be asked to complete a questionnaire and offered the opportunity to have a health check.

If you wish you may download forms here and bring them in with your proof of address once you have completed them.  You should complete a form GMS1 and a new patient questionnaire.  You will need to complete one for each member of your family if you are registering more than one person.
